  • Begin by reviewing the Terms of Use. CLICK the "Continue" button (located in the black footer at the bottom of the screen) to accept and proceed.

• Complete Form

  • Carefully read and complete each page. Required fields are marked with a red asterisk (*) symbol.

• Final Submission:

  • Once all required forms are filed, a "Click to Sign" button (located in the black footer at the bottom of the screen) will appear. CLICK the "Click to Sign" button to submit your signed document.

• Email Confirmation (Important Last Step):

  • Check your email inbox immediately. You will receive a confirmation email from Adobe Sign (adobesign@adobesign.com) with the subject: "Please confirm your signature on LTM Rising Stars - Form."

  • Click the "Confirm my email address" link within the email. This step is crucial for validating your signature. 

• Completion:

  • Once you confirm your email address, the process is complete. Adobe Sign will email you a copy of the signed document for your records.
